Standard HBO therapy does not cause any side effects. However, if the staff members perform the procedure improperly, fail to follow the operating protocols, or disregard scientific principles and arbitrarily alter the treatment plan, serious consequences may result.
1) Oxygen toxicity refers to the functional or organic damage caused by oxygen to the body after inhaling high concentrations of oxygen for a certain duration, either under hyperbaric or normobaric conditions. Oxygen toxicity can be classified into four types: central nervous system type, pulmonary type, hemolytic type, and ocular type. Regardless of the specific type of oxygen toxicity that occurs, the entire organism is simultaneously affected. Clinically, oxygen toxicity commonly results from inhaling oxygen at pressures exceeding 0.3 MPa, arbitrarily prolonging oxygen therapy durations during routine treatments, or inhaling oxygen at concentrations above 50% for extended periods under normobaric conditions. The underlying mechanisms can generally be attributed to three main aspects:
1. The toxic effects of oxygen on central metabolism;
2. The toxic effects of oxygen on enzymes;
3. Excessive generation of free radicals. Once oxygen toxicity occurs, immediately discontinue oxygen inhalation; generally, this can relieve the symptoms. Vitamin E, vitamin C, vitamin K, and magnesium ion preparations can help prevent oxygen toxicity.
2) Barotrauma: Common types include middle ear barotrauma, sinus barotrauma, and pulmonary barotrauma. Additionally, if a patient with pneumothorax undergoing decompression is not promptly identified and treated, the excessive accumulation of gas in the thoracic cavity can lead to compression of the lungs and heart, as well as mediastinal shift, potentially causing sudden death in the patient.
3) Decompression sickness: When the decompression rate is too rapid or the pressure reduction is too drastic, the solubility of gases in tissues decreases, causing free gas bubbles to form in the blood and tissues, leading to vascular air embolism and tissue compression—a high-risk condition. Fortunately, this condition typically occurs during diving operations and is quite rare in standard HBO therapy. As long as we remain vigilant and strictly adhere to established protocols, this disease is not difficult to prevent.
